IMMUNIZATION REQUIREMENTS

Immunization requirements for Transitional Kindergarten through 6th grade

California School Immunization Law requires that a pupil must be immunized against certain communicable diseases. Students are prohibited from attending school until all required immunizations are completed. All students entering TK/K through 6th grade must have the following immunizations based on their age upon enrollment:

 

4-6 year olds 7-17 year olds

Diphtheria, Tetanus, and Pertussis (DTaP, DTP, Dt, or Tdap): 5 doses

(4 doses acceptable if one was given on or after the 4th birthday)

Diphtheria, Tetanus, and Pertussis (DTaP, DTP, Dt, or Tdap): 4 doses

(3 doses acceptable if one was given on or after the 2nd birthday)

Polio (OPV or IPV): 4 doses

(3 doses acceptable if one was given on or after the 4th birthday)

Polio (OPV or IPV): 4 doses

(3 doses acceptable if one was given on or after the 2nd birthday)

Hepatitis B (Hep B or HBV): 3 doses

7th Grade Immunization Requirements

All 7th grade students are required to show proof of vaccination for all of the above listed immunizations. In addition, 7th grade students must also show proof of the following immunizations:

Measles, Mumps, and Rubella (MMR or MMR-V): 2 doses given on or after the 1st birthday

Tdap Booster (Tdap, DTP, or DTaP): 1 dose given on or after the 7th birthday

MEDICATIONS DURING SCHOOL HOURS

California Education Code 49423, 48980 requires that parents of students who need medication during school hours must do the following:

  1. Present a written statement from the student's physician detailing the method, amount, and time schedule for taking the medication including self-administered medication. This includes prescribed, over-the-counter, and herbal medication.
  2. Present a written statement from the student's parent/guardian requesting the District to assist the student in taking the prescribed medication including self-administered medication.
  3. Parent must bring the medication in the original container, properly labeled by the pharmacy (students cannot transport medication). Over-the-counter medication must be in the original box/container.
  4. In the case of self-administered auto-injectable epinephrine and inhaled asthma medication: the physician must confirm, in writing, that the pupil is able to self-administer the prescribed medication and the parent/guardian of the pupil must complete a written statement consenting to the self-administration and releasing the school district and school personnel from civil liability if the self-administering pupil suffers an adverse reaction as a result of self-administering medication pursuant to this paragraph.
  5. The written statements and authorizations for exchange of information between the school district and physician must be provided.

DIABETES INFORMATION

California Education Code Section 49452.7, requires schools to provide information regarding type 2 diabetes to parents and guardians of seventh grade students. We at Mountain View School District believe it to be valuable information for all age students. The information below was developed by the California Department of Education (CDE) in collaboration with California Department of Public Health, American Diabetes Association, California School Nurses Organization, and Children's Hospital of Orange County.

ILLNESS EXCLUSION GUIDELINES

Many times parents ask the question, "Is my child well enough to go to school?" or staff needs to determine if a child is well enough to stay in school. The Mountain View School District will follow the guidelines set by the American Academy of Pediatrics and the U.S. Centers for Disease Control and Prevention in deciding which illnesses or conditions require temporary exclusion from school.

Please review the link above for policies for temporary exclusion of students from school. You are encouraged to call the Health Office with health information that concerns your child any time during the school year.

HEALTH SCREENINGS


Pursuant to California Education Code Sections 49452-49455, students shall have their vision and hearing tested by qualified personnel authorized by the school district. In addition, students at the middle school level shall receive scoliosis screening conducted by qualified personnel that complies with the California Department of Education standards.

All students shall be tested for hearing deficiencies and for visual acuity during the kindergarten year, or upon first entry into a California school, and in grades 2, 5, and 8. The vision appraisal shall include tests for visual acuity, including both distance and near point, as well as color vision; however, color vision shall be appraised once and only for male students after reaching grade 1.

Scoliosis screening shall be conducted for each female student in grade 7 and each male student in grade 8.

Evaluation of a student may be waived at the parent/guardian's request if the parent/guardian presents a certificate from an authorized health care provider specifying the results of an examination for the specific type of screening being waived. In the event that any deficiencies are found, parents shall receive a written notice including an explanation of the health screening conducted and the results of the screening.

ORAL HEALTH ASSESSMENT

To make sure your child is ready for school, California law, Education Code Section 49452.8, requires that your child have an oral health assessment (dental check-up) by the end of the school year in either Transitional Kindergarten, Kindergarten, or First Grade, whichever is his/her first year in public school. SCHOOL PHYSICAL EXAM

The State of California requires that all students have a physical examination performed by a physician within 18 months of starting 1st grade.  If your child has a physical exam done after February 1st of the year he/she starts Kindergarten, this will fulfill this requirement.  If your child has a physical prior to February 1st of the year he starts Kindergarten, another physical exam will be required prior to the beginning of 1st grade. Pertussis Immunization Requirements for Students Entering 7th Grade

Whooping cough (also know as pertussis) is a bacterial infection that can easily spread causing severe coughing fits and missed days of school. To help protect your children and others form whooping cough, a California law now requires students to be vaccinated against whooping cough.

 

All incoming 7th grade students will need proof of a whooping cough booster shot before starting school. If your child 10 years of age and older has not been vaccinated against whooping cough, we strongly recommend getting your child vaccinated. Getting Tdap (the tetanus, diphtheria, pertussis shot) now will not only help protect your child against the ongoing threat of whooping cough but will also meet the school entry requirement.

Ask your pediatrician about other shots your child may need including:

  • a meningitis shot
  • the human papillomavirus (HPV) 3-shot series
  • an annual flu vaccine
  • a second chickenpox shot

Remember to bring your child's yellow immunization record or ask for a new one if you don't have it. If your child does not have health insurance or their health plan won't cover the cost of vaccines, call your local health department at (800) 722-4794 and ask about getting free or low-cost vaccines.
